What to Bring
When your healthcare provider orders lab work from TriCore, you may visit any of our convenient locations. Appointments are not necessary.

To help us provide you with better service, please have the following with you when you arrive at a Patient Care Center:
- Your insurance card. Please click here for a list of the insurance plans we accept.
- Your driver’s license or another valid form of photo identification.
- Your completed requisition (test order).
- Any recent changes in your personal information, including your name, address, phone number or insurance plan.
If your physician has placed an electronic order for a lab test, when you arrive please let us know that your orders are in our computer.
