How to File Insurance
At TriCore, we’re focused on keeping our processes simple, straightforward and efficient. To file insurance, we need the following information from you and your patient:
- Your full name
- Patient Name
- Patient DOB
- Patient Sex
- Patient Current Address, including City, State, Zip
- Patient Current Phone Number with Area Code
- Patient Relationship to Subscriber
- Patient health plan information, including complete and legible information on your TriCore requisition and a copy of the patient’s insurance ID card, front and back, stapled to the requisition
- Patient’s Social Security number, if provided
- Insurance Carrier details (i.e. name, address, phone)
- Subscriber Name, Policy Number and Group Number
- Ordering Physician Name and NPI
- Valid ICD-9 Diagnosis code pertaining to the date of service and meeting medical necessity (TriCore will accept a narrative diagnosis on paper requisitions)
- A completed Advance Beneficiary Notice (ABN) for Medicare patients undergoing testing that does not meet medical necessity requirements.
